Information for the National Data Base

Dear Parents,

As part of the regulation from the Ministry of Education for the national licensure and national accreditation, we must complete the national data base for all students.

In order to do this,

• we need copies of parents and student(s) passport and KITAS, for expatriate.
• we need copies of parents KTP (Identity Card) and Kartu Keluarga (Family Card), for Indonesian citizens and the birth certificate of the student(s) and student passport, if available.

Please help us to update the information in our system by sending us the copies.

This information will only be used for school purposes and will not be circulated beyond this purpose. We will make sure that all information will be kept in confidence.
